Workflow surrounding OB epidurals?

We are reviewing our current workflow surrounding OB epidurals and would like insight regarding workflows used in other hopsitals.  If you have time to answer the following questions - I would greatly appreciate it! 

Do you have MDAs, MDA residents, or CRNAs placing OB epidurals?

Do your MDAs/CRNAs enter/place their own OB epidural orders in the EMR or is this done by the bedside nurse?

 Are these orders placed before or after the epidural is in place (looking at this question from a majority of the time)?

Do your MDAs/CRNAs retrieve their own medications from an ADM for the OB epidural procedural or is this done by the bedside nurse?

Who documents that epidural medications were given in the chart? – the MDA/CRNA or the bedside nurse?

Who programs the epidural pump  - the MDA/CRNA or the bedside nurse?

Who starts the epidural infusion – the MDA/CRNA or the bedside nurse?

Do OB epidurals in your hospital require a "double check" by another RN, MDA, or CRNA?
