Our organization is starting a project for an Outpatient Medication Agreement. This would be similar to an opioid agreement or high risk medication contract between a provider and patient. We are interested in seeing other organizations processes, specifically we want to know:
1. Which patient populations receive an agreement/contract?
2. What is the intent behind an agreement/contract? What do you want the patient to achieve?

We are seeing more providers wanting to titrate paralytic agents to "ventilator synchrony", although our current titration goal parameter is TOF.

If your policy/practice allows it, could you share how you objectively define ventilator synchrony at your institution?

Where does that definition live, in policy, or is part of every medication order (e.g. default order comments)? If you are willing to share your policy or screen shots, that would be very helpful!

High alert medication storage within carousels

Does anyone have high alert storage precautions/warnings set up for meds (e.g. NMBs) in your automated pharmacy carousels? I've heard of using red tape or auxiliary stickers in the bins, but that could be difficult to see if bins are full, and hard to keep up when meds are moved around, etc. Is there any automated messaging capability? We use Talyst.
