Outpatient Medication Agreement

Our organization is starting a project for an Outpatient Medication Agreement. This would be similar to an opioid agreement or high risk medication contract between a provider and patient. We are interested in seeing other organizations processes, specifically we want to know:
1. Which patient populations receive an agreement/contract?
2. What is the intent behind an agreement/contract? What do you want the patient to achieve?
3. Is the agreement/contract in the Electronic Medical Record? If so, how is the agreement/contract shared among providers within the health system?
