Has anyone been seeing increased reactions to infliximab? We have had several reactions in the last few weeks (most minor) but it is noticeable enough that we are looking into lot #'s, etc.

Mg/Mcg mixups

I wanted to query the group to see if there were any best practices around preventing mg and mcg mixups. Specifically, if there are practices that help prevent errors associated with medications dispensed as a mg dosage forms (i.e. norepinephrine 8mg/250ml) but dosed in mcg/kg/min or mcg/min. Thank you,

Bar Code Med Admin Replacing Double checks?

BMCA is active in our facility, nursing has recommended removing nursing independent double checks in lieu of BCMA. Have others adopted this practice, using BCMA as the double check, rather than having an independent double check of the medication? Do you recommend this practice for all high risk drugs or only certain categories.

OR Irrigation Fluids

Good Morning,
We are currently evaluating our process for mixing and dispensing OR irrigation fluids. We currently mix and dispense irrigation fluids in IV bags. We are exploring changing to dispensing in irrigation bottles. Would love to hear what other institutions are doing.

3% Acetic Acid

Hi,
We were recently notified by our distributer that they are unable to fulfill our order for 3% Acetic Acid. Would love to know how others are obtaining the product or what alternatives you are using. We recently made a big push to eliminate Glacial Acetic Acid from our institution and I would love to avoid needing to bring the product back in to make 3%.

Range orders allowed?

Greetings-

We are reviewing our medication range order policy and would greatly appreciate hearing about strategies at other facilities.  I would be happy to collate replies and send out a table of deidentified responses to whoever is interested.

 

1.       Do you allow range orders for intermittent medications (e.g. morphine 2-4mg IV Q6h PRN) at your facility?                         

 

2.       If so, any restrictions?

 

3.       How does the nurse determine what dose to administer?
