Hello,

Our institution is evaluating the ISMP Targeted Best Practice of dispensing vincristine in a mini-bag to avoid intrathecal administration. Nursing is finding the change hard to accept. Are there any institutions out there that have made this change and how did you go about the change?

Newborn names

I am curious how your organization names babies after birth in the EHR. We use: MomsLastName, NBMomsFirstName. This makes it very similar to the mother's name. I am thinking something like BabyMale1 for the first name would be better. Thank you!

Insulin Pen and other Bulk medication storage

For those of you using insuling pens, I am curious how they are stored once dispensed from pharmacy. We are full Pyxis dispense but bulk items (creams/inhalers/insulin pens) are stored in med room based medication carts. I am curious if anyone has another storage option that has worked well to help prevent wrong patient errors with bulk medications. For example, storing in a locked bin/cabinet in the patient room.

Workflow surrounding OB epidurals?

We are reviewing our current workflow surrounding OB epidurals and would like insight regarding workflows used in other hopsitals.  If you have time to answer the following questions - I would greatly appreciate it! 

Do you have MDAs, MDA residents, or CRNAs placing OB epidurals?

Do your MDAs/CRNAs enter/place their own OB epidural orders in the EMR or is this done by the bedside nurse?

 Are these orders placed before or after the epidural is in place (looking at this question from a majority of the time)?

Qualitative assessement of medication reconciliation

Can you describe the mechanism in which your organization assesses the quality of medication reconciliation? We are able to capture the completion of med rec in our EHR but this is only serving as a quanitative measure and we still have see vulnerabilities in the process.

Good day all,
Does anyone still use Carnoy's solution? If yes, do you have a specific policy with regards to safe handling of this product that you would be willing to share?
We have one physician who still uses it and our Occ Health is requesting a specific handling policy for this solution be put in place.
