Calcium Replacement Protocols

Hello!

Was wondering what lab value other facilities use for Calcium replacement protocols. Ours currently tells RN to replace on either ionized Ca or corrected serum calcium.

However, RNs don't normally calculate corrected serum calcium and it seems ionized calcium is not something that's part of our main chemistry instruments in lab so testing isn't available (except we do get it in our ABGs).

What do other facilities use? Does anyone use serum calcium (not corrected)? I'd worry that we'd be prone to over correction if we did that...

Ketamine Dosing Units

Our EHR currently allows ketamine infusions to be ordered with six different dosing units (e.g., mg/kg/hr, mcg/kg/min, mg/hr, mcg/min, etc.). We are struggling to reach consensus to standardize to a single unit. Our leadership is interested in what other large health systems are utilizing as the dosing units for ketamine infusions. What ketamine infusion dosing unit(s) are allowed at your facility? Thanks in advance!

concentrated potassium vials

Good afternoon,
How does your institution/system handle potassium chloride injection for:
1- peritoneal dialysate additive for inpatients- does pharmacy prepare, or do you allow PD nurses to prepare?
2- maternal fetal medicine - for selective reduction- do you allow vials or ha?
3- cardiac perfusion- do you allow vials (with safegards??) or do you have an alternative way of dispensing
Appreciate sharing of strategies to remove potassium chloride vials from the areas.

Outpatient Medication Agreement

Our organization is starting a project for an Outpatient Medication Agreement. This would be similar to an opioid agreement or high risk medication contract between a provider and patient. We are interested in seeing other organizations processes, specifically we want to know:
1. Which patient populations receive an agreement/contract?
2. What is the intent behind an agreement/contract? What do you want the patient to achieve?

New Event Reporting Software Platform

Good Morning,

I would like to connect with others that have experience with HRP as their event reporting platform. Were you able to use the reporting forms "out of the box" or did you customize them? Was there anything that you would have done differently? Any input and tips would be greatly appreciated!

We are preparing to transition to the third reporting platform since I have been with this organization, and I have concerns that this negatively impacts our reporting. I would like to help make this transition as easy as possible on our frontline staff.

assessment of vesicants

We historically have treated amiodarone, dopamine, and dobutamine as irritants.
Lexicomp current has them all listed as vesicants.
Per our nursing policy, this would mean very frequent assessments if infused via PIV.
How does your organization treat these infusions?
