Small vs Large Hospital Medication Safety Strategies

Good afternoon,

I would love to hear you experience in implementing safety strategies to multiple sites.

Where I am, often the large academic center struggles to put things into place due to the sheer size and operational issues that they have. They often ask to opt out.

Duplicate therapy- to alert or not to alert?

I am hoping to find out if anyone has had success in turning on duplicate therapy alerts without causing too much alert fatigue?

For context, (I am on Epic) we currently have the out of the box duplicate alerts shut off, but have built a handful of high risk duplicate rules that will alert for certain drugs/categories. This doesn't seem to be enough to catch all potentially harmful duplicates.

Setting hard limits for intermittent infusions on the smart pump

Hi all,  

We are in the process of updating the limits within our smart pump drug library, and I am curious what other institutions are doing to define hard limits.  

What is your approach to setting hard limits in the drug library for intermittent infusions? Specifically inquiring about hard minimum duration limits and hard maximum dosing limits. Do you use a percentage-based rule? For example: Maximum dose for cefepime is 2000mg and establishing a hard maximum dose in the drug library would be 3000mg if using 50%.  

Respiratory meds on Alaris pump ?

Background: our respiratory therapy dept was previously using Medfusion syringe pumps for drugs in Aerogen syringes (epoprostenol, ribavirin, etc.), until those pumps were recalled a few months ago, and they were told they had to switch to a different pump. We use Alaris pumps for our IV infusions, and they switched over to Alaris and are currently running these in basic mode.

I'm wondering if anyone has experience/advice to share on this?
