Alaris Pump Keys Purchased by Patients

We have recently encountered a situation where patients are able to purchase keys compatible with the Alaris pumps through eBay or Amazon and bring them into the inpatient setting. While we can work on a process to control purchasing these keys as the replacements for the institution, what measures have you implemented to prevent a patient from using a self-procured key inappropriately?

Insulin pre-prepared doses for IV push

Does anyone stock 10unit/10mL or 5unit/5mL insulin syringes in their ADC for nurse to remove and administer for IV push administration? We are a non-24hr pharmacy, so need something safe to provide for after hours. I would love to pre-prepare doses like this instead of having them pull out of a large vial. Any stability information would be very much appreciated. Thank you!!

Hard limit monitoring with DoseTrac from BBraun

Hello,

Anyone using DoseTrac and able to obtain data on hard limit alerts?

BBraun tells us it's not possible since in these instances we had to get out of the library to be able to program the pump.

We are new users of this software and find it hard to believe there is no way to get stats on this.

Autoverified ED Orders

Like many institutions, our ED is autoverified. We do limit some orders through a "never autoverify" med list. The question has been raised about benefit of the orders still being autoverified to prevent a delay in administration, but having more orders in the pharmacist queue to review the autoverification. Looking to solicit feedback if anyone utilizes a hybrid approach such as this and how it was received.

Our EHR is Epic.

1. How does your institution handle teaching/transitions of care for patients being discharged on insulin pens? For example:

a. Are insulin pens available in the hospital pharmacy for teaching prior to discharge?
b. Are insulin demo pens available in the hospital for teaching prior to discharge?
c. Are insulin pens procured from outpatient/retail pharmacy (brown-bagged) for teaching prior to discharge?
d. Other (please explain)
e. None of the above

Tranexamic acid conversion to premade bag

RE: Best practice 20, would greatly appreciate lessons learned from folks that have transitioned to primarily premixed Tranexamic acid bags in their peri-operative spaces.

Specifically, did you encounter any challenges related to tranexamic acid topical use? Did you leave vials available in a restricted manner? Any other helpful tips.

Airway Carts in Perioperative Areas

1. Does your organization have airway carts in perioperative areas?
2. Do your organization's airway carts contain medication, specifically neuromuscular blockers (NMB)?
3. If your airway carts contain NMB, how are they stored and monitored?
