Medication Safety Committee Ambulatory

We have a large contigency of clinics and ambulatory practices within our systtem and have (relatively) recently acquired many more. As part of our new medication safety program, we are developing an ambulatory specific medication safety program.

I am wondering if anyone has done this sucessfully - and would be willing to share the structure, frequency and charter ?

our safety events are very low volume, even though medications are given in the clinics. May experience or sharing would be appreciated !

Diltiazem minibag+vial - safety concerns

Hello, our institution typically uses the premixed bag for diltiazem, however in this instance, it has been ordered but will not be received for a few weeks.

Our plan is to make the minibag+vial for nurses to activate at bedside.

There are safety concerns around this because the diltiazem is a liquid formulation.

Have you had any experience with this concern? If so, I would appreciate if you could share how you mitigated it.

AutoHeld Medications

Our nurses have expressed a concern with an EPIC workflow related to "autoheld" medications.

For those of you who use EPIC at your organization, what are the expectations for your physicians and nurses on addressing medications that were "autoheld" during a procedure & then resumed?

Interested in any staff educational materials that you or your healthcare facility has created and is willing to share on One Piece Flow/Single Piece Flow (sometimes also called continuous flow), versus batching. I am particularly interested in any pharmacy-centric examples highlighting medication preparation prior to dispensing.

BUD for IV Compounded Medications

We are currently evaluating our IV guidelines, specifically regarding the primary sources for determining beyond-use dating (BUD) for IV stability—whether to rely on manufacturing inserts or primary literature. We are seeking additional information on how other facilities establish BUD when the manufacturer's insert does not provide sufficient details.
