IV Push Premed Process

In our infusion area we have heard complaints from patients about IV push diphenhydramine and/or dexamethasone being pushed too fast. After evaluation, the nurses are appropriately pushing over appropriate durations per the literature, but this often still feels too fast to the patient. Patients report drowsiness, lightheadedness, and restless leg when drugs are being administered. Some patients receive diphenhydramine as an IV piggyback which seems to help, but this is the exception not the rule.

NICU TPN operational question

Hi everyone- Our NICU uses TPN 2000 to enter TPN orders and then these orders are faxed to the pharmacy where our pharmacists transcribe these orders into Epic. We have no available scanner in NICU currently. Does anyone else have a similar TPN process where they are leveraging Epic to avoid a manual operation?

Increasing error reporting

Does anyone have any practical tips for increasing error reporting in the hospital setting?
Above and beyond providing a Just Culture environment, making reporting as easy as possible, increasing risk awareness, thanking people for reporting and sharing changes made as a result of reporting and providing support as needed for those who self-report or are involved in an issue and other high-level ideas.
Looking for nuts-and-bolts ways to encourage people to report errors and near misses.

Pharmacy Involvement in Falls

What is pharmacy's involvement in falls evaluations in current state (whether PTA or inpatient) at your facility?
1. Criteria for who is evaluated (those admitted with a fragility fracture? those who fall inpatient?)
2. What action is taken by pharmacy (home med list or MAR evaluation? Who gets that information?)
3. Any programs looking at this for quality metrics? How do they measure? In current state our hip fracture program is looking at this quality metric.
