iLet insulin pumps

Good afternoon,

Do you permit the use of the new iLet pumps as patient home med, which autonomously calibrates and adjusts insulin delivery based on glycemic levels?

If yes, I’m interested in learning how other institutions are handling the ordering and documentation of iLet pumps, given that basal rates fluctuate, and carbohydrate counting is replaced by meal announcements. How have you structured your order sets within the EMR?

I asked about reporting medication safety work to senior leaders, which led to a discussion on sharing specific risk mitigation strategies and data related to our tools (e.g. BCMA compliance, DERS compliance, etc.).

Multiuse Medication Scanning - Manufacturer or Pharmacy Label Barcode?

We had a recent event where Flonase was left in a med bin after a patient was discharged. Another patient with the same last name was admitted to that room and received a dose from the first patient's nasal spray. Fortunately, the first patient had refused all doses, so it was still fresh, but this could have been a lot worse.

We have nurses scan the Pharmacy label barcode for insulin pens to avoid wrong patient errors, but we scan the manufacturer's barcode for other bulk items.

Mitigation Strategies with history of Malignant Hyperthermia

Does your organization have standardized mitigation strategies in the OR when a patient has a history of malignant hyperthermia?

Our anesthesia group currently does not have a standard so there is some variability in practice. Typically they will move succhinylcholine from the main drawer, or put tape over the pocket in the ADC with a note indicating MH patient. Different individuals may do more or less. I'd like to look towards standardization. Curious what others do!

Hyaluronidase for Extravasation- Pediatrics

Hello!

Curious what other pediatric institutions do in regards to preparation/dispensing and administration of hyaluronidase for extravasations in the pediatric population!

1. Straight drug (150 units/mL) or dilution (15 units/mL) for administration?
2. If dilution
a) who prepares: pharmacy or nursing?
b) all pediatric patients or just neonates/small infants?
c) if you have Epic, how is your order built out?
